What is Breast Augmentation Surgery?
Breasts are important structures that complete the body appearance of a woman. Bra sizes are determined by the cup size ranging from A to E and the chest circumference ranging from 70 to 110. Ideal breast volume is B cup, C cup is the upper limit. A and six cups are candidates for breast enlargement, and D and above cups are candidates for reduction in all breasts. When the breast volume is small, the body contour view is incomplete. Breasts may be small due to different structural reasons. One may be smaller than the other, asymmetrical. Congenitally, there may not be one at all. Silicone prostheses (implants) are the most valid method today to make both breasts equal or increase their volume. Prosthesis selection and placement depends on the patient, the condition of the breast and the surgeon's preference. Partial enlargement of the breasts can be achieved by taking the person's own tissues, for example, adipose tissue. However, since fat filling is absorbed at varying rates, it is not a permanent and predictable method.

What is a silicone breast prosthesis (implant)?


Breast prostheses are breast-shaped structures containing silicone or polyurethane on the outer wall and filled with silicone gel or saline. They are produced in various sizes according to their diameter and height (projection). There are two different types according to their shapes: round and anatomical (drop). Droplet type is preferred if a rounded shape is desired, characterized by only lower part fullness, if decollete and upper part fullness is desired together with the increase in breast volume. The surface of the prosthesis can be rough or smooth.

What you need to know before surgery


If the patient to be implanted is under the age of 40, only breast ultrasonography is performed before the operation, and additional radiological examinations called mammography are performed on it. It has been suggested that against the prosthesis, which is a foreign body, some connective tissue diseases develop in the body (connective tissue disease), it becomes difficult to give milk to the baby, and the prosthesis causes cancer. However, none of these have been scientifically proven. In breast augmentation surgery, a connective tissue layer called capsule develops around the prosthesis, which is a foreign substance for the body. In most patients, a thin capsule forms and causes no complaints. Rarely, a thick capsule may develop and this may cause hardening and narrowing of the breast, in which case it is necessary to remove the prosthesis and clean the capsule. The rupture of the prosthesis is a very rare condition, it can occur with jams, falls from height and penetrating injuries, as in in-vehicle traffic accidents. Breast prosthesis does not pose a problem for imaging methods such as mammography, USG and MRI. The prosthesis does not affect breast functions in later life, patients can become pregnant and give milk. Silicone breast prosthesis does not pose an additional risk for breast cancer, the risk is the same as normal breast tissue. When such a situation occurs, surgery and other forms of treatment for cancer are applied in the breast with prosthesis in line with the standards. Before deciding on breast augmentation surgery, it would be appropriate to clearly tell your doctor about your defect and your expectation from the surgery, to discuss the prosthesis to be selected, from which incision the prosthesis will be placed, the type of anesthesia, and the early and late results of the surgery.

Surgery


The operation is performed in the operating room under general anesthesia in hospital conditions. It takes 1-2 hours. The prosthesis is placed through a 4-5 cm incision. The incision can be placed in one of 3 different regions, most commonly in the submammary fold, less frequently in the lower part of the nipple (circumareolar) or in the armpit (axillary). The incision scar becomes vague over time. The prosthesis can be placed behind the breast tissue (subglandular) or behind the chest muscle (subpectoral) depending on the breast structure and thickness of the patient. Both applications have their own advantages and disadvantages, and this detail should be evaluated before the operation.

After surgery


Post-surgery is usually comfortable. Pain is minimal. Bleeding and infection due to surgery are rare conditions. If the prosthesis is placed behind the chest muscle, there may be limitation in arm movements for a few days. A bra or wrap is applied to the breast after the surgery. Generally, dissolving sutures are used and do not need to be removed. 10-12 if there are non-dissolving stitches. day can be taken. In the early period, there may be edema in the breast area, sometimes increased or decreased sensitivity in the nipples and numbness around the incision. These complaints disappear on their own in a short time. The patient can return to work in 3-4 days. After 3 weeks, the prosthesis settles into place and the breast regains its normal function. The patient is advised to stay away from heavy sports (which require jumping-running) for 2 months and not to lie face down. Breast augmentation surgery with prosthesis is an operation that gives good and permanent results in general, improves the mental health and psychosocial structure of the person, and makes them more connected to life.
